Our Mission

Sonder Health exists to ensure that every patient experiences a seamless, coordinated transition to the next phase of their care journey. We leverage best-of-breed technology and align incentives to make it possible for patients, families, clinicians, providers, and payers to work together without friction.

Why This Matters Now

This problem is becoming existential for hospitals. The U.S. population is aging rapidly. By 2030, all Baby Boomers will be 65 or older, creating unprecedented demand for post-acute care. At the same time, regulatory and market headwinds threaten to drive already razor-thin margins into the red. Hospitals need to do more with less, and they can't afford inefficient discharge processes that keep beds occupied.

Technology should connect, not complicate

The healthcare system has enough fragmentation. We're building infrastructure that brings stakeholders together (hospitals, post-acute facilities, and payers) on a single platform where decisions are made faster.

Discharge coordinators, case managers, and clinical teams have irreplaceable judgment and empathy. Our AI handles the repetitive, time-consuming tasks so these professionals can focus on what they do best: spending time with patients and their families.

Real-time information about bed availability, quality metrics, and outcomes data creates a foundation for smarter placement decisions. When all stakeholders have visibility into the referral process, patients get placed faster and into settings where they're more likely to thrive.
